Rich Text Format (.rtf) is readable in a variety of word processing programs. MS Word format is readable in the Word program of the Microsoft Office Suite. These forms may by completed using the word processor and must be saved using a file name which will identify you. This may then be attached to an email to a branch secretary.

Acrobat requires an Adobe Acrobat Reader. If your system does not have this program, it can be downloaded free of charge from the Adobe web site. This format, however, is only suitable for printing and filling in manually. The completed form must then be FAXed or posted to a branch secretary.
